Step 1: Your first visit

Your first visit will include up to a full hour of 1-on-1 time with a member of our licensed medical staff. You’ll get a physical, health screenings and plenty of time to talk about your health questions.

Step 2: Regular visits

After that, you’ll receive regular in-home visits with UnitedHealthcare At Home either at least once a year or more frequently with ongoing medical care in your home. It all depends on your needs, and we work around your schedule.

Step 3: Ongoing support

You’ll work with your care navigator to help coordinate your ongoing care needs. They can also connect you with information on resources that may be available to you, like transportation services.

No, UnitedHealthcare At Home is included for people enrolled in an eligible health plan. There’s no additional cost to you for this service.

No, having a UnitedHealthcare At Home visit won’t affect your benefits at any time.

UnitedHealthcare At Home provides a yearly in-home visit, which includes:

  • A head-to-toe exam
  • Health screenings
  • Plenty of 1-on-1 time to talk with a member of our licensed medical staff about your health questions

In addition, UnitedHealthcare At Home provides ongoing support with a care navigator who will work with you help you coordinate your care needs — from scheduling appointments to finding local resources and more. Based on your care needs, you may also have access to ongoing medical care in your home.

Because each individual’s needs are unique, resources and information provided will vary based on those needs.

A member of our care team who is either a nurse practitioner, physician assistant or case manager. For ongoing medical care in your home, you will see a doctor, nurse practitioner or physician assistant. All UnitedHealthcare At Home professionals are state licensed and maintain national certification. In addition, our health care practitioners have passed a background check.

  • UnitedHealthcare At Home supplements your doctor’s care with an extra set of eyes looking out for your health between regular doctor’s visits.
  • Your in-home health care practitioner will help you by supporting existing relationships with doctors — and can also help you establish relationships with new care resources, if needed.
  • Doctor’s appointments can sometimes feel rushed and you might not know exactly what to ask. Our care team can provide you with a checklist of important questions and topics specific to your needs to discuss with your doctor at your next visit. This helps ensure you get the information you need.
